FOCUS ON HEALTH

Grooming & Hygiene

Provides medically oriented services to individuals who have a chronic, stable condition that can be safely maintained at home. Personal care visits include bathing, hair care, oral hygiene, shaving, dressing, assistance with toileting, help with walking or transfers to wheelchairs, meal planning and preparation, assistance with self-administered medications, and other health related items. Depending upon the needs of the client, a Nurse follow-up visit may be recommended at scheduled intervals for a needs evaluation and care revision.

Medication Administration

We will send a caregiver to ensure that medications are taken as prescribed. Medications must be set up ahead of time by a family member or our nurse.

Special Diet Assistance

Preparation of special diets to meet an individual’s needs, such as cardiac, diabetes, or renal issues. Menus can be created and food prepared to account for sodium, fat/cholesterol, and sugar content.

Bath Visit

A bath visit is helpful to make sure that someone is receiving the encouragement or assistance they need to take a bath or a shower. Bathing is sometimes very difficult and is responsible for quite a few falls related to limited mobility or difficulty transferring. They are one hour in length.

Exercise Assistance

Assistance following an exercise routine developed by a home health physical or occupational therapist. Ongoing support is provided to help ensure a successful outcome and improve activity tolerance.
