What is the Medicare GUIDE Program?

The GUIDE Model is a nationwide initiative designed to:

  • Improve quality of life for people living with dementia
  • Support family caregivers with real resources
  • Help seniors remain safely at home longer

The program is no cost to families and is covered by Medicare with no co-pays or out of pocket expenses.

Dementia Care Services Covered by Medicare GUIDE

Through Focus Healthcare and partners like Harmonic Health and Continuum Care eligible families receive:

A nurse practitioner will meet with your family virtually, every 6 months, to help assess needs and connect you with valuable resources to help your loved one stay at home longer.

We provide around-the-clock help for urgent questions or concerns.

Focus Healthcare empowers families and reduces caregiver stress by providing Alzheimer’s and dementia education and support coaching.

Medicare’s GUIDE program helps caregivers take much-needed breaks by coverering $2,500 in annual support for in-home respite care or adult day services. This amount is equivalent to approximately 73 hours of care.

A dedicated Focus Healthcare Care Coordinator will help you navigate the Harmonic Health system, Medicare benefits, additional funding sources to reduce the cost of in-home care, and available community resources. They may also help coordinate medication review and management and home safety recommendations.

Who Qualifies for the GUIDE Program?

Eligibile Families

  • Have traditional Medicare (Part A and B)
  • Care for a loved one with suspected dementia or confirmed dementia diagnosis (e.g. Alzheimer’s, Lewey Body Dementia, vascular dementia, etc.)
  • Live at home or in a community setting, including independent or assisted living

GUIDE is Not For Those

  • Currently in hospice
  • Enrolled in Medicare Advantage plans, including Special Needs Plans (SNPs)
  • Living in a Long-Term Care or Skilled Nursing Facility
  • Enrolled in Medicare’s Program of All-Inclusive Care for the Elderly (PACE)
